PCN Practice Nurse

This is a fantastic opportunity for a experienced Practice Nurse to join a genuinely forward-thinking and well respected PCN in South Somerset. The PCN is looking for a Practice Nurse to have a key influence over the home care provision of QOF, vaccinations for housebound and care home residents and management of other staff such as HCAs, GPAs and SNAs.

You will also support practices within the PCN with capacity issues and provide cross over where challenges arise, so there will be a need to travel between practices as part of the role as well as getting involved in home visits.

The PCN are looking for a Practice Nurse, who is comfortable undertaking all general nurse duties (chronic disease management qualifications would be desired but not essential) who is happy to have a varied role between practices and home visits. Salary up to £22.17 per hour DOE and an opportunity to work with a very well respected PCN.

Salary –  £34,000 – £43,200 per annum DOE (£17.40 – £22.17 per hour DOE) + NHS pension + 26 days Annual Leave + Bank Holidays + Indemnity

Location – South Somerset

Your role –

• Provide clinical leadership for the vaccination program across the PCN
• Be comfortable handling all general nursing duties (chronic disease management experience is desired but not essential)
• Provide clinical leadership and support to improving the delivery of QOF across the whole PCN.
• Show and provide leadership across the PCN to the wider team – GPAs, HCAs and SNAs.
