Meet our winners! We are delighted to announce the Primary Care Stars for October to December 2024! We had hundreds of nominations for October to December so it was very hard picking just a few winners!

Sybil Montague, Practice Nurse at North Wood Group Surgery

Sybil was nominated for “identifing a patient with non symptomatic atrial fibrillation, due to thorough checking. She noticed high blood pressure and an irregular heartbeat, so arranged a hospital ECG. The patient is now on medication to manage the risks. The consultant said the patient was a stroke waiting to happen.”

The whole team at Pallion Family Practice

The whole team were nominated because: “A young patient had cardiac arrest. The team came together but not just with the care of the patient. The Practice Manager and a member of staff went to the patients home to get his wife. Young children where at home so the staff member stayed with the children in their home to allow the wife to be by her husbands side. The practice acted fast both int he practice as well as ensuring the family were also cared for. They did manage to safe the patients life using a defib.”

Meet our Highly Commended Stars!

Sheikh Mateen Ellahi, GP Elm Tree Medical Practice

Sheikh Mateen Ellahi was nominated as Elm Tree “started to branch out into community projects such as going to local libraries to give talks on health, CVD risk, Alongside this we have also bid to provide more public health awareness regarding cancer in the local area, especially to those that are disadvantaged groups, such as asylum seekers, people who do not have english as their first language.

Dr Sheikh Mateen Ellahi has been instrumental in organising this and has been the main driver in this preventative approach. Alongside this he is the main driver allowing for us to continue to provide such high quality care, even though the surgery is growing at 1000s of patients per year.”

The Reception Team at Family Medical Centre

The Reception Team were nominated because they “are brilliant at their jobs. We have recently changed several ways in which we work, and brought in a new telephone system, online bookings and so on, and they have been so helpful, patient, and have taken on all the new changes like absolute champions.

They are an integral part of the team, supporting the clinical staff tirelessly, helping patients out all the time and we often hear great feedback. It’s a job that often gets a lot of difficult encounters because of the nature of GP in the NHS – and they handle situations with grace and understanding, always quick to do whatever they can.”

Deb Lister, Ash Grove Medical Centre

Find out why Deb was nominated: “she was appointed as Practice manager in March 2023 the surgery was in ‘requires improvements’ and we were due a CQC inspection. Deb turned the surgery around and we were awarded ‘good’ overall and Godd in ALL areas – this is the highest rating the surgery has ever got. Deb has gone on to motivate the whole practice team and worked tirelessly to raise moral to all our staff.”

Anam Abrar, GP Ancora Medical Practice

Dr Abrar was nominated as she “exemplifies excellence, compassion, and innovation in her role as a GP, consistently going above and beyond for her patients, colleagues, and the wider community. Her leadership in the “Know Your Numbers” campaign included training 20 community members to take blood pressure readings.

This culminated in a highly successful BP campaign on September 5th, where over 60 BP checks were conducted across community locations, increasing awareness and promoting preventive health measures. She has empowered her team by educating staff and HCAs on hypertension management, building confidence and capability across her practice.

Her dedication to patients, her team, and the community makes her a true Primary Care Star. Dr. Anam Abrar is a shining example of compassionate, innovative, and outstanding general practice, and she is incredibly deserving of this award.”

Sarah Morton, Practice Nurse and Georgina Gee, Trainee Nurse Associate Hawthorn Medical Centre

Sarah and Georgina were nominated because “Post Covid Hawthorn surgery was left with a long list of patients that would need spirometry for diagnosis. At the same time, there were no members of staff able to perform spirometry and interpret the results. Sarah and Georgina set upon the task with appropriate training they were able to work together to get the service up and running.

Working together as a team they are inspirational to the cause, are providing a quality service to the patients, and have gained respect from the wider team at Hawthorn by knowing these 2 individuals can be called upon to help with advice for staff and patients alike. I believe these 2 are worthy of an award to recognise all they have done for the patients at Hawthorn with respiratory conditions.”
