    Background

This is a permanent frailty GP role built around continuity of care: a defined group of older, housebound and care home patients, looked after by one team that gets to know them well. There is no duty doctor rota, sessions can fit around school hours, and you do not need a formal special interest in frailty, just a real interest in caring for older people. Recurrent funding means this is a long-term post, not a fixed-term pilot.

    Location

The role covers a mix of coastal, market town and rural communities across mid Cornwall, with south coast harbours and open moorland close by. St Austell is around 20 minutes away by car, Truro around 35 minutes and Plymouth around 50 minutes.

    The Practice

    • Permanent post within an established neighbourhood frailty team, backed by recurrent funding
    • Experienced multidisciplinary team including a senior nurse lead, frailty nurses, care coordinators, an occupational therapist and a pharmacy technician
    • Short daily team huddle and a monthly complex case MDT, with community and supporting services involved
    • GP colleagues provide additional sessions across the week, so you are not the only Doctor cover
    • Real scope to shape how the service develops as it moves towards more proactive care

    Your Role

    • 4 to 6 sessions per week, with Mondays and Wednesdays as core days (half or full days)
    • School hours pattern available, roughly 9am to 2pm spread across the week
    • No duty doctor commitment
    • Responsive work, including acute reviews and home or care home visits where a case needs GP input
    • Proactive work, including care planning, comprehensive geriatric assessment and admission avoidance
    • Triage shared with the frailty nurses
    • ACP and senior nurse supervision, teaching and quality improvement
    • The role flexes with your sessions: more sessions means more hands-on visiting, fewer means more senior advice and coordination

    Salary and Benefits

    • £11,595 to £13,668 per session
    • Permanent contract
    • 6 weeks annual leave plus bank holidays
    • 1 week study leave
    • Indemnity paid by the practice
    • NHS Pension

You must be qualified as a General Practitioner and registered with the GMC and UK performers list.
